Accident

Objective: To report occurrence of any accident within the plant during day-to-day activity. It allows user to report, review, investigate and approve/reject accident report using the system.

Below steps to be followed to complete an accident report:

  1. Report Accident : To create/report new accident report. Any employee working on the site, who has access to the system can report an accident.
  2. Review & Identify investigation Team member : Review Accodent Report & Identify investigation Team.
    • Plant HS Team User shall receive the accident report for review and identify investigation team members. Either one of the HS team shall take up the review task.
    • Plant HS Team includes Fire Fighting, EHSMS and Admin, Health, Safety Role.
    • Any user can be selected as Investigation Team member. (At least one Plant HS Team member must be selected)
  3. Accident Investigation : Investigation will be conducted by any user who is selected as investigation team member. User can submit CAPA with investigation report if required.
  4. Review Accident Investigation (HOD) : The investigation report will be reviewed by “Head of Department”.
  5. Review Accident Investigation (Head of Division/ Deputy Division) : The investigation report will be reviewed by “Head of Division/ Deputy Division”, if report is generated with category of "Fatality or Serious Bodily Injury" Or "More than four (4) days Lost Time due to Injury or Illnesses"
  6. Review Accident Investigation (Plant HS Head) : Once “Head of Department” approves the report then investigation report should come for approval of “Plant EHS Head”. if report is satisfactory then report can be sent for acknowledgement to EHS MR, otherwise the report will be sent to investigation team to revise.
  7. Acknowledge Accident Investigation (EHS MR) : Once “Plant HS Head” approves the report then investigation report should come to of “EHS MR” for acknowledgement. Accident report will be closed upon acknowledgement.

Note :

  • Any of the user from the “Fire Fighting, EHSMS and Admin, Health, Safety” roles can review the incident Report and identify investigation team.
  • At least one Plant HS Team member must be selected in investigation team..

REPORT ACCIDENT

  1. Go to Accident | Report Accident screen.
  2. Enter the Department, Contractor/Sub Contractor, Location/Sub-Location, Exact Location in auto complete text box and enter Event Date and Time. (Department, Contractor/Sub Contractor, Location/Sub-Location, Exact Location will be displayed automatically when first 3 characters are typed)
  3. Click “Submit” button to save the data and send for review to “Plant HS Team”.

Note :

  • Accident can be reported by any employee working in site.
  • The fields marked with * are mandatory to be filled.
  • Employee must select “Plant”from drop down list if the employee Is not belongs to any plant.
  • Automatic notification alert will be triggered once incident is reported.
